1. Registration and MR release form

We need to know a little bit about you. Please fill out this form for general information including age, church info, medical info,  photo release, etc. 

2. Camp Rock Creek Release form

Download the release form in the link.  This must be filled out by a parent/gaurdian and brought to camp or emailed to Luke. Camp Rock Creek gives no exceptions.


3. Pay for Camp

This link takes you to our Giving page to submit your payment. Downpayment is $95 or full payment is $195. Choose the "Camp Rock Creek" fund and list the camper's full name and T-shirt size in the payment description.

4. Info Sheet

What do you need to bring to camp? Check out this page for theme dress-up days, dress code, camp policies, and all the other needed info for camp!
